Home Assessment Reablement Team (HART) is a community based service delivering support to patients leaving hospital and clients already in their own home that need additional support to prevent hospital admission.


Home Assessment Reablement Team require a full time Administrator working predominantly from the John Radcliffe Hospital. You will be required to work on a rota basis which will include working weekends.

As the team Administrator, you will be responsible to the Head of Service and will work closely with the Assessors and Reablement Support Workers within the team. You will be required to have a good knowledge and understanding of IT systems and general administration duties.

This post is key to the referral process into HART and the operational coordination and administration of the service.

The post holders will be joining a multi professional team and would be ideal for those with a strong administration background. Enthusiasm and flexibility in approach to work are essential.


Close links have been developed with our community colleagues to ensure a smooth handover to appropriate services for our patients and to facilitate a smooth pathway for our patient’s ongoing care. As the Administrator for the team, you will be required to work with our extended community and health based colleagues.


Supervision, support and comprehensive training will be offered to ensure that the service delivered to the patients is both of a high quality and meets both Trust objectives and patient needs.
